 format and type in the school or group
name at the top
with full contact details please.
When completed with all your entries please email back to me and
send your cheque made payable to MHMCT to the address on the entry
form

On behalf of my team,
I certify that we are medically fit enough to run the event, I agree
that the organisers will in no way be held responsible for any loss,
damage or injuries to ourselves or our belongings before, during or
after the event. I understand that Personal Data provided to the
Organisers is protected by the Data Protection Act and will not be
used for any purposes other than this event.
